CONTRACT & HEALTH WARRANTY

This agreement is between (Seller) Laurie Taylor and:

(Buyer) __________________________________________ / __________________________________________

It is agreed between Seller and Buyer as follows:

1. DESCRIPTION OF DOG

The dog sold pursuant to the terms of this agreement is as follows: Sex: ___________ Whelped: _____/_____/________

Color: _____________ sired by __________________________________ out of __________________________________

2. CONDITIONS OF SALE

Seller guarantees that at the time of delivery, the dog is in good health. Should dog be found to be in poor health by a licensed Vet , Seller will provide buyer with a replacement dog from another litter after dog is returned to seller at buyers expense, or

a full refund of the purchase price of dog only. Shipping fees will not be refunded.

This dog is sold on a limited AKC registration. This dog is sold for pet only and not for breeding purposes.

The following conditions apply to the sale of the aforementioned dog:

A. The Buyer has THREE (3) days from the time of receipt of dog to have dog examined by a veterinarian. Any

request to return the dog to the Seller for a full refund must be made within this time period and may only be

requested should the dog prove to be seriously ill upon arrival. Proof of examination must be provided to seller.

This guarantee will be terminated immediately if dog is not examined within the allotted time.

B. A warranty against serious life threatning genetic disease is provided for two years after the date of sale.

C. A warranty against HD is provided till the dog reaches 25 months old.. The guarantee for HD will be terminated

should dog be allowed to become overweight at any point during the warranty period. Should dog be found to

have HD within its first twenty five months, buyer must provide seller with proof in the form of documentation from the

Orthopedic Foundation for Animals or from Pennhip. We reserve the right to have a veterinarian of our choice

confirm the results before a replacement is made.

D. Warranties shall apply only to original purchaser. Transfer of dog ownership shall void all such provisions of this

agreement.

E. Under no circumstances shall the Seller be liable to the Buyer or to any third party for a dog which has been

subjected to abuse, accident, negligence or misuse. Also, under no circumstances shall the Seller be liable for any

consequential incidental or special damages resulting from or in manner related to the dog.

F. Upon execution of this contract all veterinary care and its associated costs are the sole and exclusive responsibility of the buyer.

3. MAINTENANCE

Buyer agrees to maintain this dog in good health, provide routine preventative health care including, but not limited to,

Inoculation, internal and external parasites, and preventative medication. Veterinary records must be provided to breeder if

requesting a replacement. If any of the above preventative care has not been provided, health warranty will be void.

4. FULL AND FINAL AGREEMENT

The parties agree that this writing represents the entire Agreement between them and that no other representations, either oral

or written have been made regarding the dog described above. The parties further agree that no changes in this contract shall

be binding upon either of them without written modifications signed by both parties.

If this agreement has not been signed and RETURNED within 10 days of delivery of dog to buyer, then warranty is void and breeder is released from all contractual obligations.

Buyer agrees that he/she understands this contract fully.

In case of any litigations the State of Georgia court system shall have rule over this contract.
